一、资助类别、期限及授课语言
I SupportingCategories, Duration and Instruction Language
1.资助类别。本科生和硕士研究生。
1. SupportingCategory.This scholarshipsupport undergraduate students and graduate students.
2.资助期限。本科生5学年，硕士研究生3学年。资助期限为录取时确定的学习年限，包括专业学习和汉语补习的期限。
2. Duration. Thisscholarship only supportsbachelor’s students for no more than 5academic years,master’s students for no more than 3 academic years.The scholarship covers bothmajor study and Chineselanguage/preparatory study, as specified in theAdmission Letter.
3.授课语言。一般为汉语

The ChineseUniversity Program provides a full scholarship whichcovers tuitionwaiver,accommodation, stipend, and comprehensive medicalinsurance. Please refer toIntroduction to CGS—Coverage and Standard for detailsof each item.
三、申请条件
III Eligibility
1.非中国籍公民，身体健康；
1. Applicants mustbe a citizenof a countryother than the People’s Republic of China, and be ingood health.
2.学历和年龄要求：
2. The requirementsfor applicants’ degreeand age are that applicants must:
●来华攻读学士学位者，必须通过汉语四级考试，年龄不超过22岁
be a HSK 4 Level Certificate holder under the age of 22 when applying for the bachelor degree;
●来华攻读硕士学位者，须具有学士学位，通过汉语五级考试，年龄不超过35岁；
be a bachelor’sdegree holderwith a HSK 5 Level Certificateunder the ageof 35 when applying for the master’s programs.
四、申请流程
IV Application Procedure
1.申请人向山西中医药大学国际教育学院留学生管理办公室提交申请。
Step 1 - Applydirectly toInternational Students Office.
2.登录“国家留学基金委来华留学网上报名系统”（登录http://www.csc.edu.cn/laihua或http://www.campuschina.org后，点击“网上报名学生入口”图标进入）填写申请信息，在线提交并打印《中国政府奖学金申请表》。受理机构编号为：10809（必填）
Step 2 -Complete theonline applicationprocedure at CGS Information System（Visithttp://www.csc.edu.cn/laihua orhttp://www.campuschina.org and click“Application Online” to log in）, submit online thecompleted Application Form for Chinese GovernmentScholarship, and print a hardcopy.Agency Number is10809（Required）
3.在规定时间内将申请材料提交至山西中医药大学国际教育学院留学生管理办公室807室
Step 3 - Submit allyour applicationdocuments toInternational Students Office（office 807）beforethe deadline.
五、申请材料（均为一式两份）：
V Application Documents（in duplicate）
1.《中国政府奖学金申请表》（中文或英文）；
1.Application Formfor Chinese Government Scholarship（writtenin Chinese or English）;
2.经过公证的最高学历证明。如申请人为在校学生，须另外提交本人就读学校出具的在学证明。中英文以外文本须附经公证的中文或英文的译文；
2.Notarized highestdiploma;
Prospectivediplomarecipients must submit official document issued by your current schoolto proveyour current student status or expected graduation date. Documents inlanguagesother than Chinese or English must be attached with notarized Chineseor Englishtranslations.
3.学习成绩单。中英文以外文本须附经公证的中文或英文的译文；

Transcriptsinlanguages other than Chinese or English must be attached with notarizedChineseor English translations.
4.来华学习和研究计划。用中文或英文书写（不少于800字）；

Thisshould be aminimum of 800 words.
5.推荐信。来华攻读硕士学位者需两名教授或副教授的推荐信，用中文或英文书写；
5.Two RecommendationLetters（written in ChineseorEnglish）;
Applicantsfor the master’s programsmustsubmit two recommendation letters signed by a professor or anassociateprofessor.
6．《外国人体格检查表》复印件。登录国家留学基金委网站http://www.csc.edu.cn/laihua或http://www.campuschina.org下载，英文填写。申请人应严格按照《外国人体格检查表》中要求的项目进行检查。缺项、未贴本人照片或照片上未盖骑缝章、无医师和医院签字盖章的《外国人体格检查表》无效。检查结果有效期为6个月；
6.Foreigner PhysicalExamination Form（photocopy）（written in English, can bedownloadedfromhttp://www.csc.edu.cn/laihua or http://www.campuschina.org）;
Thephysicalexaminations must cover all of the items listed in the ForeignerPhysicalExamination Form. Incomplete forms or forms without the signature oftheattending physician, or the official stamp of the hospital, or asealedphotograph of the applicant are considered as invalid. Please carefullyplanyour physical examination schedule as the result is valid for only 6months.
7.已获得HSK成绩报告的，请将复印件附在申请材料中。
7.The copy of validHSK Certificate（ifavailable）；
8.护照首页复印件（首页、内白页）；
Copies ofPassport's First Page and blank Pages；

9.FORM FORPROVISONAL ACCEPTANCE OF INTERNATIONAL STUDENT (MASTER) BYSHANXIUNIVERSITY OF CHINESE MEDICINE
以上材料请按上述顺序在左侧装订成册（一式两册）。于2019年3月15日前交至山西中医药大学国际教育学院留学生管理办公室807室无论录取与否，申请材料均不返还。
Alldocuments shouldbe bound together on left in the order of 5.1 to 5.8.（Youshould submit TWO sets of bounddocuments）. Submit allyour application documents toInternational Student Office（office 807）before the deadline March 15，2019.Noapplication documents will be returned.
